Proof of Severe Disability

All applicants must submit a statement from their physician that includes their disability and diagnosis. Your application cannot be processed without proof of severe disability.

Proof of Tennessee Residency

All applicants must submit proof of current residency annually. Proof of residency must show the applicant's name and address. Documents must be dated within the last 60 days and must be an original (no copies).

Proof of U.S. Citizenship

The State of Tennessee requires that we have proof of citizenship for all Family Support Applicants. Please submit a copy of your birth certificate with your application.
